We ARE a provider for regular Medicare and Anthem Senior Advantage.

We provide the finest in eye care at an affordable cost. We participate in most health plans and accept assignment on Medicare claims. You are responsible for any deductible, co-payment, and non-covered charges. Payment for services is required at the time of visit.


We are NOT a provider for United Healthcare.  (Call United Healthcare and ask they place us on the provider panel, if you would like to see us become a participating provider).


COVERED SERVICES (Ohio):

FOR:  Senior Citizens, age 65+, or under age 65, patients with  SSI Disability Insurance,

FOR:  Vision Examination for the purpose of evaluation & treatment of established ocular medical diagnoses, such as cataract, glaucoma, macular degeneration, diabetic retinopathy; or evaluation, treatment and management of PATIENT COMPLAINTS for symptoms such as dry eye, ocular allergy, loss of vision, red eyes, crusty eyes, headaches, eyestrain, flashes/floaters, etc.

EYEGLASSES - one pair following cataract surgery, for people with intraocular lens implants (diagnosis: pseudophakia) (one pair glasses following right eye cataract extraction, and one pair following left eye cataract extraction).  Only basic eyewear is covered.  NON-COVERED items include deluxe frames (cost over about $60),  invisible bifocals, most lens options.  You will be advised at the time of your order what items are not covered.  For diagnosis of surgical APHAKIA (cataract extraction without intraocular lens implants), you may be eligible for one complete pair of eyeglasses each year.  In addition, there may be coverage for MEDICALLY NECESSARY contact lenses for a limited range of diagnoses (aphakia, for example).


These policies are subject to change by your Medicare Carrier.  These policies are for regular Medicare, NOT HMO plans.  Check with your insurance carrier for final determination of benefits.

There is NO COVERAGE for "ROUTINE EYE EXAMINATIONS" or routine EYEGLASSES.
