Frequently Asked Questions About Release of Medical Records 1) How do I obtain a copy of my Spectacle or Contact Lens Prescription ? Simply call our office and we will gladly mail you a copy of your current prescription. You may obtain a copy of your Spectacle Prescription for up to 3 years after your examination date, without charge, even though the prescription may be expired. You may obtain a copy of your Contact Lens Prescription for up to 2 years after your examination date, without charge, even though the prescription may be expired. If another office requests your prescription, you must provide a signed release of your information. The parent or guardian must sign for minor children. Please be sure to include the following information: patient's name, birthdate, address, and telephone number, to help us properly identify the correct record. In addition, the provider must supply us with the name, address, telephone, and fax numbers where the information is to be sent. Please allow 48 hrs. to process your request.
2) How do I obtain a copy of my Medical (Examination) Records ? Your Medical (examination) Records, which contain your personally identifiable health information, is subject to HIPPA regulations. We REQUIRE a signed authorization to release this information, ON OUR FORM designed for this purpose. You may obtain a copy of this form by calling our office. The Release of Records form will be posted here within a few days, to download in PDF format. Please allow 48 hrs. to process your request.
3) Why is there a charge for release of my records to another office ? First, we sincerely appreciate your patronage, and hope you continue to choose us for your vision care needs. If, however, you choose to see another provider, there is no charge for obtaining a copy of your spectacle or contact lens prescription (see above FAQ's). Your medical records, however, remain property of Suburban Vision Care, Inc., and are kept on file for seven years since your last office visit. The fee, which is defined by Ohio Revised Code, section 3701.741, covers our expenses incurred for the storage and retrieval of your examination records. Costs associated with the storage and maintenance of your records include rent, heat, lights, insurance, security system, telephone system, and staffing, as well as other overhead costs.
